Aetna, Anthem, Assurant, Blue Cross/Blue Shield, Cigna, Humana, Medicare, Pacificare, Principal, Unicare, and United are accepted.

At this time, we do not accept Medicaid, Scott & White or Worker’s Compensation.

Appointment info:
Patients are seen by appointment only. If you must cancel or reschedule, please let us know at least 24 hours in advance to avoid a cancellation fee. Please be prompt for your appointment so we can provide the best possible care to you and others.
